Good interpretation, but they are not all "metaphors." Like the Joker Smile is an allusion, but good job nonetheless.
But for the Joker thing, I thought the stretched smile at 2:43 is an allusion to the Joker "Let's put a smile on that face" This was during a time (2007) where her label tried to hide all the bad press by making Britney appear "happy", but it was fake and she knew it, thus a fake smile.
