Streams were not included, the RIAA has confirmed this.  Ariana is just a case of overcertification. This also happened recently with Yeezus, which was certified Platinum, despite being well short of this sales threshold.
 
RIAA does not use streams to certify albums. This may change in the future though.  |

It's a high-profile example of over-certification still sometimes happening in the modern era. It used to happen a lot. Technically it should be a lot harder to do now, cos digital sales never overship and they provide real-time sales receipts, plus Nielsen Soundscan system provides more accurate data than the previous method.
 
I don't know how they made a mistake for her album. The RIAA did confirm though they didn't change their methodology to incorporate streams, because people were asking them due to this Plat cert unexpectedly coming out of nowhere.     |
