 In my opinion, they did everything right during TF and TFM eras. They were being extra meticulous. They didn't want to tarnish her image in any way. And that's why she quickly became the most powerful pop star in the world. If BBB had been released as the 5th single as planned and it failed to go Top 10 (which would have been likely), things would be different. 
They really just did everything perfectly. The four best TF single choices, Paparazzi to Bad Romance transition, the social media and online marketing, TMB, the performances, the videos - she literally had the best team in the world back then. 
Of course, it also helped that the music was fresh and innovative and catchy and nothing like anyone else was doing at the time. The Fame was literally a phenomenon back in 2008. People were catching on to Lady Gaga left and right. People adored her. Just Dance, and then Poker Face - it was just crazy. I'm really not sure if she's capable of it again, but she certainly has the opportunity now more than ever.
