The general thing is that the movie takes place in 2056 and there was a huge organ failure but a company found ways to give replacement organs. If you can't pay for them though, they get repossessed. For people like Paris' role, its cool to get the newest liver, heart, etc. In the movie she has a new look every scene, she is the company's CEO's daughter so she gets everything free. What zydrate is, is like a pain killer that makes the surgery painless. So I guess like a drug.
