I think Gaga does have impact in all of those areas. You might not see her impact on fashion by what the public wears, because who can actually afford to dress like Gaga with all those costumes? Those pieces she wears aren't what you'd wear outside. It's not even about originality and self expression. No one can possibly be comfortable wearing what she wears for a couple all day.
At a certain point everyone is music was trying to be like Gaga through their sounds and image. That is impact. She has impact on art if artists are influenced by her.
Forgetting about this, you can't really visualize someone's impact until much later. Five years is hardly enough time.

when madonna popped up on the secene there were immediately look-a-like contests (judged by andy warhol),
the madonnaland at macys,
thousands of young girls dressed like her,
and a bunch of singers who tried to immitate her holy mousie voice.
and that's what i call IMPACT.
