I don't know what triggers it TBH. I know when I started lifting weights and eating a lot more(including a **** ton of milk) my left nipple got puffy/hard like.

Raise your arm and pinch around your nipple. If you feel a rubbery mass around your nipples, you need surgery. No amount of diet and exercise will fully get rid of it. If you don't feel a rubbery mass, just fat, you can get rid of them with diet and exercise, or liposuction if you have the money.
Most males experience "man boobs"through their teenage years and it goes away in a few months to a year. If you're still dealing with this issue in your 20s then it's classified as Gynecomastia. Otherwise it's just puberty.
