That still seems the most likely scenario; whether Frank takes the heat for Fiona in a moment of genuine concern for his daughter, resulting in Fiona in a genuine altruistic moment agreeing to the transplant surgery, or whether Frank will use the offer as a bargaining chip, it seems like the writers are gearing up to this (and it will most likely be a mix of the two).
Frank realistically and thematically needs to die by the end of this season, or if it gets renewed, by the middle of next (which would most likely be the last I would guess); his body is shutting down, even surgery isn't going to save him at this point (but it will perhaps prolong his life long enough for him to enjoy a drink at the Alibi by season's end).
The depiction of his illness has been chilling so far, the scene in the Hospice and Frank's breakdown were hard to watch.
