Here, I made this.
What people thought was a pillow Gaga was resting her head on, I think is actually a large, ruffled collar, like the one Pierrot is wearing. Also, it looks like it's in black and white, but actually Gaga has white makeup on her face, much like a lot of Pierrot characters do.
It could all be wrong, but it's interesting and I surely hope she goes down this road. I love her referencing old art.

And it's not a drawing, Gaga is drawing OVER the artwork, the artwork is a real picture.
Also, Helen does not work with Gaga, she works for Gaga.
