It's either a very complicated metaphor for her being dropped, or indication of an abortion. Since the former happened after the hospital scene in the video, and Gaga said the intro was very structured and linear, the hospital visit being a metaphor is very unlikely. She'd also said she was going to reveal a dark part of her past she hadn't previously shared, and we already knew about her being dropped.
However, it could also be her just ****ing with us.
