I think Vogue and Like a Virgin refer to how she is trapped by her fame:
The caged corset is supposed to portray how Madonna the person has been caged into this larger than life persona: Madonna the Queen of Pop (which is represented by the corset), She then takes it off for Candy Shop and Human Nature and she then has a breakdown in Like a Virgin and when she is vulnerable on the floor crying, this guy (dancer) comes a picks her up and restrains her tightly into a new corset to a point where she can nearly not breath, like saying: This is your job and now you have to do it! You can't escape your fame! You wanted this, so now GET TO WORK!

The tour remix of that song actually made the album version listenable for me. And YES YES to the Masculine/Feminine section! On of the best she's done in ages.

And it ties in with the poem she wrote and read on the 'Im going to tell you a secret' documentary that said "I have a cage and it's called the stage..."
Tbh I am not sure she wants to keep doing this, I think artists in the music industry are to some extent forced to continue working for the huge corporations even against their will and when they are useless (they loose their voice, they are a drug mess, they can't sell anymore) they are killed for more profit.
