Well, that's based on a lot of factors... let's take a couple of examples...
1) GREAT CALLOUT, that means people are really excited about the song, requesting it like crazy.
2) CONTROVERSY, the song may be particularly strange or controversial like... Gold Digger, Hollaback Girl, and others.
3) TV HYPE, best example: American Idol. AI has helped Kelly get airplay and obscure artists like Daniel Powter get a lot of airplay just because his song was played on the show.
4) ARTIST HYPE, if the artist is big enough, radio will just add their songs without question and if the song is good enough it will rocket to the top.

5) LOCAL ARTISTS, artists from big metropolises attract even a whole STATE to play the song, like Teairra Mari, Michigan played "Make Her Feel Good" OUT and it became a top 40 hit, and her album debuted at #4 on the charts.
6) UNDER THE TABLE BRIBES, usually for artists that record companies feel they put lots of time and effort on but songs just aren't catching on
7) CLUB SONGS, probably one of the most popular reasons, any song that can be played in the club is a song that has BB success potential, slower songs can be valued as more authentic, but I'm sure we all know that "Check On It", "Laffy Taffy", "I'm In Luv (Wit A Stripper)" and "I'm Sprung" have NO reason being as high as they were.
