There's this thing that I didn't quite understand so well: She was forced to have the lobotomy in a week starting from the moment she got into the asylum, like you said. So, the '1st imaginary world' was created right from the exact moment before she was lobotomized or was it being created as the real events were happening as well? I know you said that "just a split second before the operation is performed, she retreats into an imaginary world all inside her head", but damn it's hard to believe that all that story would happen in just that second, you know? I wasn't expecting the movie to be realistic at all, but those are the little things that mess with my head.
Sorry if I seem to be kind of dumb right now, but I even got Inception much better at first watch.

Honestly, I don't think it matters whether she was in her imaginary world the whole week or just right before she was lobotomized. It could be either one, and neither case hurts the story. The only reason I say she retreats into an imaginary world just before being lobotomized is because that's how the movie portrays it and because well to me it makes perfect sense that way as well. It's like how they say your life "flashes before your eyes" before you die, or like how you can be asleep for five minutes, but have a dream that lasts way longer. So as I understand it, before being lobotomized she just totally went into her own little world.
They need to hurry and put this on DVD. I will be snatching it up asap.
