No it doesn't all mean the same thing. Clean means lack of dirt or grime. Uninfected means not harboring a pathogen. Use uninfected or negative.

Because I think referring to people with HIV as "unclean" or "dirty" is beyond rude. Positive people are still people. It's no different than having hepatitis or cancer.
