Monarchy was abolished not along ago, 1946 to be exact, so no, he didn't proclaim himself the king of Italy out of nowhere, his dad was the last king before Italy became a republic.

You guys are stupid, obviously Italy isn't a monarchy today but the history of the peninsula is filled to the brim with various noble families dating back to the Middle Ages. The last Shah of Iran and his family members are still considered royalty too, even though Iran clearly isn't a theocracy
