An understanding of context would have done you a great service before you posted this, not limited to music, but to culture as well. We're discussing artists whose avenues for reaching an audience were limited to television, radio, and print media. And I'm not sure if you took a gander lately, but all of Africa isn't safari, people living in huts, and zebras coming up to the window while you're in the drive thru at McDonald's (believe it or not, yes, we have McDonald's here! ).
Not having access to internet, radio, and TV doesn't contribute to why Madonna isn't popular here because several artists were popular here without those mediums. Madonna just isn't one of them.

He and his team ran with it as a great marketing tool.
Wether Madonna or any other female that has been called The Queen of Pop organically or not doesn't matter. It has and will always be just a play off of The King of Pop as the media felt it needed a female equivalent.

Madonna's never cared about her QOP title, she rejected it back in 2012. "Don't confuse art with aristocracy." It was given to her naturally and not by her team.

And it's a title that's never in contention. Just like The King of Pop, The Queen of Soul, The Godfather of Soul etc. The Queen of Pop is literally the only title in music that's always in contention.
