The only thing Netflix said is that around Spring Time that they'll release the Disc. Yes, you need a disc to get Netflix up and running on the Wii.

I believe you need your on account, so you'll still be charged.

It shouldn't be an extra fee. You can stream on any current "Netflix-ready" device on your TV as it is now at no extra fee. As long at you pay at least for the $8.95/month unlimited plan, you can stream for free.

Currently the PS3 (for everyone) and xbox (for gold members only) allow streaming for free so it shouldn't be any different here.

Of course, you should be aware that you can't stream every movie. You won't find many new releases to stream instantly, however the selection is not bad and it is basically just a bonus for being a member anyway.
