Actually, let me put it this way; would you bet his life on it?
In short, if your friend says that he wants to kill or hurt himself, and he doesn't sound like he's joking, always,
always take it seriously. You can't help him if he's dead.
That said, don't keep feeding into his habit by saying nice things about him when he's fishing for compliments. If he hates the way he looks, maybe he needs to do something to change it. Encourage him to work out, or suggest some places he could shop for clothes.
Also, if you're his "only friend," then he needs to make more friends. Tell him to get off his ass and start putting himself out there. Or invite him to larger social gatherings and introduce him to some of your friends. He's clearly not happy with the life he has right now, so he needs to take steps to improve it.
