The New York Daily News is reporting that shots had been previously fired at the home (and I imagine the police were aware of this) and that she didn't obey orders to put down the gun.

Did they bring the officers up on charges?

I honestly think there's really an inordinately high amount of people seeking careers in law enforcement for all of the wrong reasons and there needs to be extensive psychological testing to vet and weed out these types.

There are many Police Officers that get their jobs after Military Service. And in Military Service if you ever feel you are endanger you just shoot to kill.
