He probably suffers from depression which caused this heavy weight gain. To get out of that black hole is an achievement by itself. Hopefully he's not surrounded by people that pressure him to lose weight and tell him it's not ok the way he is. That only leads to a dead end eventually. I mean, of course it's not okay, but that's only gonna make things worse, which happened for him, unfortunately. Support is the better choice, not rejection.
