The truth - just say you need to take some time off for personal reasons; if they request specifics just let them know that you are getting burned out and it's compromising your health and will effect your work if you don't look out for yourself.
If you manage your time well and have a good attendance record, performance and work ethic, this should not be an issue for any reasonable employer.

3 days is a lot to take for a job you've only had for four months, I've never had a job that I could miss 3 consecutive days and not require some sort of explanation from a doctor or something, or a notice of a funeral.
