There's no confirmation about that. All we know is that she had both twins at some point in the asylum and that Boone was raised poor and that he doesn't consider Gigi his mother. It's very likely she put them both up for adoption.
The theory isn't that Grace is the twin, but she's the final RD. The other twin is one of the girls in the sorority who is completely innocent and unaware that she's related to the killers. Grace was convinced she was a twin from the tub by Gigi and bringing her to the cause. When in truth we know her mother was Bethany the sorority leader who had her exactly nine months AFTER the twins were born. Making her close enough to the same age as Boone for her to believe that she could be his twin.
