You'll look different if you work out 3 times a day, eat a balanced diet, and sleep well in 12 weeks. If you're trying to lose weight, and can successfully lose 1 to 2lb a week, then 12 to 24lbs of body fat gone is noticeable. If you're trying to gain muscle, and can successful gain 0.5lbs a week, then 6lbs of extra muscle is noticeable.
I don't know if you are expecting people to go from 115lb twinks to 230lb bodybuilders, because that's obviously not achievable in 3 months. But basic results are.
