The scariest one I've seen is Shakira's fall back when she did the Tour of the Mongoose at Madison Square Garden...one of the panels on her stage collapsed when she stepped on it and her face hit the floor face first & bounced off (while she was playing a harmonica...could've easily busted her mouth open/broken several teeth). She was able to pull her leg out from underneath the stage & get up while continuing to play the harmonica...a true professional...
