Think someone who looks, acts, talks, and has mannerisms like P!nk but is in fact a homosexual female.
And no I don't think Butch lesbians get treated worse than fem gays for example, because masculinity is the norm in today's society. You'll probably be labeled cool by straights. However within their own community they might not get as much action as their lipstick counterparts would because they tend to look for feminine lesbos.
It's basically the same thing as it is with masc gays.
