It's not that people don't know Madonna is in her 50s (which isn't really "old" but whatever, I guess it is if your life expectancy is short but considering her impeccable health and the care she gives to her body, it's only about half of her life). It's that the ad implies that old people are inherently gross. It ties into how society views older people at large, and how they are discriminated against when it comes to employment, are often forgotten by lawmakers and things like the mainstream gay rights movement (which cares a lot about marriage but not about what will happen to those married couples once they retire and need to find LGBT-friendly homes and such), there is a generalized culture of disrespect and neglect towards older people.

ageism and racism aren't comparable

someone can be ageist, but it's not like their never going to be 60 one day. Someone can be racist, but they will never ever wake up one morning and be the race they are prejudist against.
