The song was written for several of her gay friends who died of AIDS, all of which are mentioned in the song.
Just because she doesn't explicitly say the word "gay" doesn't mean it's not a gay song. It's arguably a song about AIDS foremost, but it's still definitely more LGBT conscious than whatever her contemporaries were putting out at the time.
