I'd rather have it be refresh-based, especially for more popular threads like when a main pop girl releases a lead single. It'd get too hectic otherwise.
Yes, mainly because it takes so damn long. Especially on a busy award night or album release, when all those guests are just PerchedT it's just not cute.
Having to wait 7 minutes with a 15% chance of actually loading the page is just too much on days like those, I'd rather not be able to access the site all together than suffer the annoyance.
