Ok, but here's the major difference: a quarterback has only seconds to analyse the defense and make sure his offense is properly in place and micro-seconds once the play starts to decide how he is going to run the play before the defense takes him down. That's the difference.

Ok, first of all they don't run non stop, they slowly jog a lot of the time. Also, I won't say some midfielders don't have a lot to think about. They are professional athletes in that position for a good reason and I highly respect them for that. However, my opinion rests on the time the quarterback has to make so many different decisions. There is only about 5 elite quarterbacks in the world. Think about that, many athletes around the world try out for NFL positions, there are many former soccer players that are kickers in the NFL, there are a few former rugby players in the NFL, but there are only about people capable of being elite quarterbacks in the world. That to me says a lot about how hard it is to play such a position.
