There are tons of shows and movies where contestants and actresses have passed. They usually dedicate the season to them in the beginning of the episode. Like the Real Housewives of Beverly Hills, when season two was finished filming, Taylor Armstrong's husband, Russell, committed suicide. They addressed the suicide in the beginning of the episode, and proceeded on with the show, I guess you could say a little 'flashback' scene before the 'show' begins (which, I heard they did a lot of editing, because season one, Russell wasn't really edited out as a good person).

It also happened with a contestant on The Bachlorette this year, where his parachute collapsed. They made a tribute to him on an episode.
