The difference is playing with a doll is natural for all children. All children do play with dolls regardless of gender, and in most cases it's alone in the house where nobody else can see. Wearing a dress especially outside is making a statement that some people may disagree with.
Is it wrong that people disagree? Yes.
But it's better that than my child having to go through the hurt of being made fun of for wearing the dress.
It's more painful to be teased by peers, or even worse adults that may see him outside than for I the parent to just say "a dress isn't appropriate for school" .
If it is dressing feminine is what they want you could do a counter-offer and propose for them to wear a pink shirt or something like that so they won't get teased but won't feel unexclusive.
