You can never be sure. Just recently I watched a sex video where there was a twink fem boy with lipstick on and a muscular bearded guy. One would think that it is obvious that the muscular guy would top the fem boy. But it was the opposite: The fem boy topped the muscular one. That was super awkward to watch but the fem boy was kinda good at topping.
