Whilst Gaga gets irritating with her grandiose waffle, you get that she bleeds passion. She ****ing LOVES this. For someone to adopt such an energetic and enigmatic persona indefinitely - you'd have to. She lives, breathes and sleeps fame, music and theatre. Plus, strategically, there's a lot of self-fulfilling prophecy going on. Associate "Lady Gaga" with "art", "talent", "music", "costumes", "fashion", "passion", "performing" etcccc. enough (regardless of reason), and guess what she becomes synonymous with? Have a think what her contemporaries are associated with.
Regarding fans, her actions speak louder than words - and that's saying something. On the marketing side of things, she has created a lifestyle brand that thousands of fans willingly buy into. It's not just music - it's fandom, it's an experience and it's a community. It definitely has its downsides (crazy obsessives, entitlement, alienating casuals etc.) but she invested enough into it all early on that she can largely just sit back and reap the rewards now. The ARTPOP era being a testament to that.
Whilst it may seem OTT, bizarre, and less immediately rewarding to some record labels/acts, they'll see the downside of pandering to the fickle public instead of establishing this brand and following when things go sour.
