Well since Madison said when she died, it was all dark and empty. So I assume at the last moment Fiona talking with Cordelia and showing some type of love towards her - showed she had a soul and that put her into hell. Except it was her own personal hell - stuck in eternity with axelman in a little shack, reliving the same life over and over again.

Well that's what I got from that scene.
