I'm not justifying what happened, NASA should've canceled the launch but under the circumstances it was understandable (still doesn't make it right) and they had no intentions of killing any one. It was a fatal mistakes, but mistakes soddenly happen, they weren't as advanced back then as they are now and NASA had contributed so much to our science, pointing fingers at them and belittling them as "mediocre" isn't the way to go. They didn't attack Beyonce after all.
Second of all, I don't think its a big deal that Beyonce used the sample. Personally, though, I don't think it goes with the song and would've fit more on a song like "Heaven" (considering the concept).
