There was no real "mistake", she could have released Bad Romance now, it would still have flopped because of the collateral damages from last era. So to me the "biggest" mistake is that it came after the BTW era.
There was a lot of time from first single to album release date. I thought they'll release the second single's video by that time. But they didn't. First single's hype lost, and second single's hype hasn't happened yet.
Does the confusing direction have anything to do with it? Her tweets about it throughout late 2012/2013, the constant change in imagery, the promotional shoots having nothing to do with the actual album, the music not matching the imagery and concept, the fact that the concept kept changing and still changes if she's asked about the title now, etc??
I think the biggest mistake was release Applause as first single, though the single was a hit in US, it failed to catch up in globally, she should've gone with DWUW as first single since it's better, catchy and the lyrics are somehow relatable. I'm not sure if the break hurt her tho.

But like you said, Applause was a "hit" in the US and yet the album still underperformed there.
So at the end of the day, I don't even think it was the lead's fault, to me the GP just got tired of her.
