James Colbourn, 27, from Ipswich is nominated by his younger sister Sadie Colbourn, 23, from Wolverhampton for all the support he has given her.
Sadie gave birth to twin boys George and Riley at just 26 weeks. A year later they were diagnosed with cerebral palsy and James has been by her side every step of the way. James gave up his spare time and cared for the boys like they were his own and Sadie wouldn’t of coped without all the help from her older brother, and wants to show how grateful she is.
James receives an unexpected surprise at The Savoy in London from pop star sensation Britney Spears.
Also on the show, a 91-year-old man takes to the ice for a special performance with ice skating legends Torvill and Dean, a huge Cher fan gets a special message she will never forget and there is a special Christmas performance from Olly Murs.
And Michael Búble delivers a very special message to two fearless ladies who raise money for the nurses at Macmillian plus a lady is reunited with her three sisters who live in Australia.
