According to witnesses, the lap bar was secure in the station, most likely the problem was due to body type. You can be overweight and ride, but it all depends on how you're shaped.
If you're top heavy and the restraint is on your stomach (which seems likely) it was just a freak accident because this ride is known for it's high negative g forces). You're supposed to be strapped in by your thighs, but when the ride threw her up, her body shape changed due to the forces and she was thrown out.
