The photo is clearly just an international show of support for a fairly horrific event, not some political statement on bombings that take place in other countries constantly. If she wanted to portray that, she'd use a photo of a bombing victim in another country, subtlety is not Madonna's strong suit.

Exactly, I hardly think we're supposed to be taking this message as sarcasm

Hey last week was a bombing in irak, 100 deaths or so, are you interested?

Well, it's only a matter of reason that the more geographically close an event occurs, the more likely it is that someone is going to know about it or pay attention to it. I think there's even been studies done on this. If 5 people die in a car crash in my neighborhood, I'm going to be more interested and aware than if it happens 2000 miles away on the other side of the US.
I'm not saying people shouldn't care, but it's hard enough for people with jobs and/or school to even keep up with all the news in their own country, let alone be well versed on all international events. There are more than 6 billion people on this Earth and it's quite an unfortunate fact that a lot of them do terrible things to their fellow human beings. It's a lot to keep up with and people shouldn't be talked down to for not knowing the latest in bloodshed around the world.
