You could try download mp3tag, after that. Just put the folder of an album and then Select all and paste the cover.. It works faster

What you got to do is...
On your computer where all your music is located you have to remove the artwork and then you right click and press get album artwork. It should find them automatically on iTunes. Then you just re add the songs to your iPhone. But make sure you delete them first from your iPhone then resync them. It should work it happens to me on my ipad.

This happened to me once after I synched my music with a different computer than my own.. Not sure why, but eventually it was normal and I don't remember how. sorry for not being helpful. I would recommend re-synching it?

Restart or re-install iTunes. Works for me.
