The quality is usually the same if ripped from the real CD or itunes onto blank discs, the cover is different or the same with poor quality, some bootleggers that put effort into their bootlegs would actually have a booklet of all the artwork/descriptions/tracklist/etc. like the actual CD in the stores but that's rare nowadays.

DVD's are usually recorded from a hidden camera at the actual theater as the bootleggers are watching the films themselves.
