I like it when people go crazy with them, and go very unconventional.
first one I can think of off the top of my head was Thom Yorke's the eraser. the actual packaging itself was like some weird cardboard thing, and inside when you folded it all out you got this picture:

my favorite is actually from a mexican girl group called Jeans.
They made it look like you got a package in the mail, with cardboard and lasso and junk. Inside the "package" there was the CD and 13 postal cards, each postal card had a picture of them and the lyrics to one of the songs on the CD, the 13th card had the thank you's and credits. It was soo cool!
There is so much good album artwork and as a fan of detailed fantasy art, it's hard to choose one because there is a lot of it. Conversely though, the one that most appeals to me is quite simple but poignant.
Yeah it does I guess, probably because in certain video games (e.g. ones that had an ancient/exotic feel with characters like Soul Blade/Edge) use fantasy art and others know it as goth/comic/biker art. I love album covers from bands like Iron Maiden, Iced Earth, Rhapsody of Fire, Symphony X, Nile, Epica, Engima etc. They all have really graphic art pieces - I just don't like any that are vulgar.
