I think the stat you're all referring to is that 1 in 5 (20%) of sexually active gay men in
urban areas are infected. That's from a 2010 study by the CDC, which only sampled men in cities. It's extremely difficult to get a full picture of HIV among gay men in rural areas, as they are much more likely to be living with the virus without access to testing, treatment, or care. There's also no real pattern in most rural areas. For example, rural Missouri has a very low rate, but rural Louisiana has an extremely high rate.
