You have to have an agency first and foremost; they submit you to casting directors for runways/shoots/campaigns. It's rare for a model to get a campaign, but when they get one big one, it obviously opens doors for others. Your success can be dependent on the climate of casting, so the nineties/early 00s saw both really bulky and really skinny models, followed by more classic models (like Gandy), then followed by models who have more unique looks. I mean, the people cast for Saint Laurent and Dior will be different than Dolce & Gabbana and Versace, so there's a market for everything. Nowadays, you have the opportunity to market yourself through social media and become successful, though you have to stand out (ie LBS and his bleached hair).
Honestly, with scouters using Instagram and fashion houses/casting agents browsing Tumblr, the rise (and fall) of model can be really quick. Long story short, find an agent.
