Golden Triangle tickets were only won through contests. There were contests through Icon, her fanclub. There were some that if you had a Madonna related tattoo and a ticket you could enter, there was also if you had a ticket you could take a photo and enter. In stadium shows where the size of the Golden Triangle was going to be much larger they had a lottery where a woman walked by with an iPad and you would touch it and possibly win your way in.

Fever wasn't lipsynced I don't think. But yeah Erotica and Vogue were. She's got a long history of lipsyncing the Vogue performances, I think I read around the time of the Re-Invention World Tour that she does it because of the complex choreography she does it's hard to do both.
