It has nothing to do with dying at all.
It's about a relationship coming to an end. "The Last Song" is a metaphor of how their relationship will be remembered. It's quite obviously about her and Chris' break-up.
First verse: They're at the breaking point of the relationship. "Mid-air off the cliff" is symbolism for that scary moment when one of them finally ends the relationship, making it final that it's over. She goes on to sing that maybe it's not necessarily a bad thing that they're breaking up, but that they could instead have fond memories of their time spent.
The chorus is her wondering if she'll be able to do that. After a relationship is over, you tend to think of only the bad things and forget about the good. She's wondering if that will be their 'Last Song' (ie memory of eachother) as opposed to a positive one.
2nd verse: Describing that, despite how much the relationship hurt and broke her, she'd still do it again because something positive came from it. 'Maybe it'll save the world' = maybe someone else suffering DV will learn from what happened. 'They're gonna miss hearing it' = reference to friends and fans wanting them to get back together. 'It's time to turn on...' = referencing the media frenzy the relationship got. Basically admitting she had no choice in the matter. The public broke up with Chris for her.
The 2nd verse is urging both herself and Chris to think of the good times in the relationship. The hook is her looking back on it fondly, and even keeping the possibility of them getting back together open. A very clever way to end the personal CD.
Looking back, I realize she he actually forewarned us, ya'll.
