I think that Nicki's outburst at Miley was preplanned, either by MTV encouraging her to do it or by Nicki herself, but I think Miley genuinely didn't know she was going to say it and her reaction was real. So it wasn't completely fake imo.

Seeing as that Blurred Line pic was already lined up I think Miley's response was already planned.

BUT I think the plan was for Nicki to accept her award, and then Miley to throw a little bit of shade at nicki being a sore loser. And then nicki becomes the butt end of a joke.

Nicki must have caught wind of this plan, and threw the 1st punch, just in case.

Notice Nicki's mic was cut off, so she would have been defenceless to anything Miley would have said after collecting her award.
