Accept her at her word that the song is about fear of suffocation, and the metaphorical meaning of the video is pretty obvious. She's suffocated for the entire seven minutes.
She's in chains. She's in a jail cell. She had cigarettes over her eyes and can't see. She's in a tiny, cramped cab of the ***** Wagon with huge hats that make the cab even smaller. She's surrounded by product placement (and then stifled by American flag imagery everywhere). She has the telephone hat that covers part of her face, and then the telephone hair that does the same thing. They have those giant burqa things on that cover their bodies. The police are closing in at the end.
Suffocation, EVERYWHERE, which totally fits with the song's meaning about wanting to escape a stifling relationship. (Beyonce killing her boyfriend, anyone?) The ENTIRE Telephone video is metaphorical.
