It definitely does exist, I see a lot of it in the gay fetish communities I'm involved in (though of course I never participate myself). It's actually linked to one of the most common fetishes you come across, but is just a very extreme and taboo expression of the fetish that most are unlikely to reach at a young age before years of desensitisation and seeking harder, darker thrills opens them up to it.
Chances are it does get blown up by the media to appear more prevalent than it actually is, because that is the nature of shocking phenomena and the media's fascination with such things. But it is certainly there and it is not so small a % of the community as you would like to think, at least from my own experiences.
