Incest doesn't automatically cause genetic mutations. It's a problem when there is a hereditary health defect that would get concentrated through incest whereas breeding with a non-family member will more likely dilute the gene.
Take for example a "carrier" of a certain bad allele. That's when a person has a disease or sickness in their genes but they don't have it. They are simply carrying it. If they mate with someone who's not a carrier of said allele, there is a very low chance the disease will be passed on. And if the family keeps breeding with non-carriers the chances of passing on the disease get smaller and smaller.
But let's say you breed with a sibling. You are both carriers so your offspring will have a higher chance of getting whatever genetic disorder runs in your DNA.
