And if you think you can only afford fast food, you're just not smart enough with your money. It's not like it costs anything to exercise and go for a run either.
It doesn't need to go directly into the healthcare system.
It goes into other areas as well, like as fire engine services and such.
20% isn't a heavy tax.

no, not all of them are but almost all of them. i've talked to a lot of obese people and it's not like they want to be obese or that they woke up one day and were like 'what a nice day to become obese'. there is always something more to their story.
i don't really see what's so hard to understand about it? especially in todays economy many people struggle financially. eating healthy on a budget can be done but is very hard, involves a lot of planning and is still going to cost more.
let's take the salad comparison above (i am aware that there is loads more healthy food out there then salad but since people are claiming it's possible to make a salad cheap, let's compare)
Pizza = 0.99c --> 750 calories
Coke (no name brand) = 0.55c --> 139 calories
Total calories: 889
Total cost: 1.54
Salad:
1 butterhead lettuce = 0.79c --> 21 calories
1/2 pack cherry tomatoes = 0.99c --> 13.5 calories
1 medium sweet potato = 0.34c --> 102 calories
1/2 cup quinoa = 0.65c --> 111 calories
Total calories: 247.5
Total cost: 2.77

and if it doesn't go directly into the health care system then your argument of it making more money is irrelevant to the topic.
and it is a heavy tax because it adds to an already hefty price. according to 2014 research my country was the 4th most expensive country in Europe.
